Athletics Overview

All student athletes must meet the following requirements to participate in our athletic program.

• Minimum grade point average (GPA) of 2.0
• Currently not receiving a grade of F in any class.
• Currently receiving a satisfactory citizenship grade. No unacceptable (U) report from a teacher or administrator.
• Submitted all required signed parent permission forms to the coaches (See Forms section below.)

LeyVa Middle School does not charge a participation fee for athletics. Our sports programs are supported through fundraising efforts by students and staff, which help cover the costs of coaches, transportation, game officials, and equipment. To help sustain our program, we ask student-athletes to consider making a $25 donation to the athletic program.

We encourage all students to participate in athletics as a meaningful way to build friendships, develop teamwork, and create lasting middle school memories. No students will be denied participation due to financial circumstances. Families with questions or concerns are encouraged to speak with a PE teacher or team coach.

These forms must be completed and submitted to the coach before participating in any sport.

Voluntary Accident Insurance for Students

Who should consider Student Accident Insurance?

What if my child already has health insurance?
This plan acts as secondary coverage. It can help pay what your main insurance does not cover—such as deductibles, co-pays, or benefit limits.

What if my child does not have health insurance?
This plan becomes primary coverage. It helps cover eligible medical expenses directly, giving families a safety net for accident-related costs.

Why consider it?
Accidents happen at school, during sports, and even outside of school. This low-cost insurance helps protect families from unexpected medical bills.
